Step 2: Consider any new therapy options that will help you achieve your health goals.

Step 3: Determine the total number of selections you would like to commit to per month to optimize your health.

Step 4: Each month use consistent selections OR mix it up from month to month to meet your wellness goals.

Important Information

  • Consultation, exam and X-rays prior to starting the membership is required for new patients | $99.

  • Selects may roll over from month to month but expire after 3 months.

  • A minimum 3 month commitment is required.

  • Selects must be used within 3 months after cancellation, or they will expire.

  • Families with minor children and spouses may share a plan.
    *sharing only available for 5 & 10 Select Memberships

  • Auto draft payment: Membership fee will be applied on the first Monday of every month.
